GP Online services
GP Online Services gives patients the option to make appointments, order repeat prescriptions and view their electronic medical record online, using their computer, tablet or smartphone rather than having to phone or visit their practice.
Wem & Prees Medical Practice recognises the benefits of GP online services and is committed to enabling patients access in an efficient manner whilst ensuring that appropriate checks and safeguards are in place to ensure that the interests our patients are protected and that there is no misuse of the system.
Most general practice services rely on varying levels and methods of identity verification. Access to online services demands a more consistent and robust approach to ensure patient confidentiality while providing them with secure access to personal and sensitive data. A secure identity verification process is required before full access to appointment, repeat prescription ordering or record access services may be enabled for a patient. Every Practice is required to verify patient identity documentation, or individually vouch for each patient requesting access to online services. Proxy access can also be obtained and refers to access to online services by somebody acting on behalf of the patient and usually with the patient’s consent. To obtain formal proxy access a person must register at the Practice for online access to the patient’s record, though the proxy does not have to be a registered patient at the Practice.
Before new patients register for access to their practice’s online services, it is important that they understand that they will be responsible for protecting the information about them that they will see, please read the patient responsibility policy carefully along with the patient information leaflets below.
The NHS App provides core record access which gives patients the ability to book & cancel appointments, request repeat prescriptions, view any recorded allergy and also view your COVID vaccination status. The App is easy to use and does not require patients to attend the Practice to verify identity, this is all completed through the app using a smart device (smartphone, ipad, laptop, PC with webcam etc) and acceptable photo ID such as a passport or driving license. If you would like to apply for access to GP online services please complete the relevant application form below and bring this to the surgery along with appropriate identity documentation which must include photo identification and proof of address. Please Note these must be 2 separate identifiers and 1 piece of identification documentation cannot be used for both i.e. driving licence shows picture and address however this can only be used as one form of identity and either another piece of photo ID or proof of address will be required.
To ensure your identification request can be dealt with promptly we are asking, where possible, for patients to visit the Practice on a Wednesday afternoon. If you are unable to do this we do hold some evening and weekend clinics during which a staff member will be available to complete the verification process.
